Senior Social Worker (Supported Housing Coordinator)
Duties and responsibilities include, but are not limited to:
Develops working relationships and agreements with other organizations and directly coordinates with the designated Public Housing Authority (PHA) regarding housing voucher application and award process for each veteran. Regularly conducts and participates in outreach activities including field interviews, assessments and referrals for homeless veterans contacted in the community, shelters, and on the streets, as well as those who are referred by VA residential programs, medical centers or outpatient clinics. Evaluates the veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion including an assessment of vulnerability and priority for admission. Assesses at-risk factors and develops a preliminary plan, involving the veteran and family or significant others, and performs an insightful assessment of serious and complicated cases involving psychiatric illness which may also include, catastrophic medical condition, dementia, traumatic brain injuries, and other high risks diagnoses. Provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. Develops psychosocial treatment plans, actively involving the veteran and their family or significant others, in coordination with interdisciplinary team members, based on the psychosocial assessments, including goals for clinical treatment. Using known resources, the incumbent makes initial and continuing decisions coordinating the veteran's care through linking and referral to VA medical facilities or Regional Offices and other VA and non-VA services and provides independent consultation and makes recommendation to the interdisciplinary team on course of action. Provides direct mental health and substance abuse services for veterans and family members or significant others in support of the veteran's treatment. Gives advice, guidance, emotional support and other assistance and provides individual and group counseling services as well as crisis management services needed to maintain the veteran safely in his or her residence. GS-12 Senior Social Worker Experience, Education and Licensure. Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgement. At least two (2) years should be in a area of specialized social work practice. Senior social workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.In addition, the candidate must demonstrate the professional KSAs below:
Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Assignments. Senior Social Workers are licensed or certified to independently practice social work at an advanced level. Senior Social Workers typically practice in a major specialty treatment program area, such as a Polytrauma Rehabilitation Center or Polytrauma Network Site; a Spinal Cord Injury Rehabilitation Center, or a national VHA referral center, such as a national Center for Post-Traumatic Stress Disorder or a national Transplant Center, or other program areas of equivalent scope and complexity. The Senior Social Worker is assigned administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns. They are also assigned in settings where they have no access to social work supervision, such as CBOC'S or satellite outpatient clinics. They may be assigned to work with special patient populations with highly-complex health or metal health problems requiring expert level psychosocial interventions, such as VA/DOD liaisons assigned to Military Treatment Facilities as part of seamless transition or social worker case managers in Mental Health Intensive Case Management (MHICM) programs. The full performance level of this vacancy is GS-12. Physical Requirements:
The work requires extreme patience and controlling of emotions. The work may required using appropriate techniques to physically restrain patients who present a danger to self or others. The work may also require assisting physically disabled Veterans. Day-to-day work involves sitting, walking, standing and carrying light objects. The work is typically performed in a clinical setting that exposes the incumbent to the communicable diseases common to the homeless population. Must of the work is performed in a setting away from the office, where the environment cannot be controlled. The incumbent must be able to safely operate a government owned vehicle for the purpose of community work and transporting patients. References:
