GOVERNMENT OPERATIONS CONSULTANT I

APD TACACHALE

GAINESVILLE, FLORIDA

Ensures residents receive various medical, habilitative and programmatic services based on their needs.
Investigates, assesses, analyzes and makes recommendations to resolve issues and problems.
Provides daily consultation and assistance to the Rehabilitation Therapists, Behavioral Program Specialists, and supervisory staff in the operation of service programs and responsibilities.
Consults with program managers and supervisory staff to resolve programmatic, administrative, and operational issues and problems.

Oversees proper admission and discharge of residents to and from facility.
Schedules Individual Support Plan (ISP) meetings within 14 days of admission and ensures that existing programs are run during the interim period.
Coordinates the work of interdisciplinary teams to develop ISPs.
Ensures ISP meetings are held annually for each resident.
Completes quality ISPs and distributes them within the specified deadlines.
Supervises the implementation of the ISPs and reviews the residents' progress with the interdisciplinary teams monthly or as assigned by the facility.
Addends ISPs with the documentation submitted by the service providers.

Maintains compliance with State, Federal, and Medicaid regulations.
Ensures required standards are met.
Develops and implements plans of correction as needed.
Ensures through monitoring the achievement of corrections to deficiencies in accordance with established guidelines.
Schedules in-services for home staff as needed to ensure effective delivery of residential services, provides for the safety, health, and welfare of residents; and ensures compliance with rules and regulations.
Monitors home activities, off-home training sites, and center-wide activities as required by the facility.
Meets with guardians and guardian advocates as needed.
Attends psychiatric clinics, facility, and center-wide meetings as required.
Prepares various reports as needed.

Ensures residents' safety through the review of all incident reports.
Initiates preliminary investigation and completes a 5-Day Report of Findings as determined by the Program Operations Administrator or designee.
In the event of Level 3 injuries or as directed, hold a Special Interdisciplinary Team meeting within 24-hours from notification of the incident or the next business day.
Coordinate follow up actions with the interdisciplinary team and ensure completion of the corrective action plan.

Performs other related duties as required.

MINIMUM REQUIREMENT(S)

A valid Florida Driver s License (If initially hired with an out-of-state license the Florida resident must obtain a valid Florida State driver s license within 30-days of hire.
License suspended or revoked for any reason, work permits (Business purpose/ Employment/ Education only license) and some types of restricted license are not acceptable.
License that have Corrective Lenses Restricted, are acceptable, provided the driver wears the corrective lenses while operating the vehicle.

PREFERRED ADDITIONAL REQUIREMENT(S)

A Bachelor s Degree and one (1) year Training experience, and one (1) year experience working with Persons with Disabilities.

Proficiency in Microsoft Word and PowerPoint, recording and editing for multimedia presentations.

This position may be required to perform essential services before, during, and after an emergency per APD OP 2-0035.
This position is critical to the safety and supervision of residents and may require the incumbent to work beyond their normal work schedule to maintain minimum coverage levels in the facility.
